1. HIPAA Privacy Policy:

Patient Confidentiality and Privacy

Fit Bella complies with the Health Insurance Portability and Accountability Act (HIPAA) to ensure the privacy and security of your protected health information (PHI). Our complete HIPAA Privacy Policy is available upon request. We take the utmost care to safeguard your personal and medical information.

2. Appointment Policies:

Appointment Scheduling:

  • Appointments can be scheduled by calling our front desk at 469-400-8424 or through our online booking system.

No-Show Policy:

  • We understand that life can be unpredictable. However, if you are unable to make your scheduled appointment, please notify us at least 24 hours in advance. Failure to do so may result in a no-show fee.

Late Arrival:

  • If you arrive late for your appointment, we will do our best to accommodate you within the remaining time. However, your treatment time may be shortened to avoid delays for other clients.

3. Cancellation Policy:

Cancellation Notice:

  • To cancel or reschedule an appointment, please notify us at least 24 hours in advance. This allows us to accommodate other clients and adjust our schedule accordingly.

Late Cancellation/No-Show Fee:

  • A fee may be charged for late cancellations (within 24 hours of the appointment) or no-shows. The fee amount will be communicated to you at the time of scheduling.

4. Consultation and Consent:

Consultation:

  • A consultation may be required before certain treatments to assess your needs and ensure your safety.

Informed Consent:

  • Before any treatment, you will be provided with information about the procedure, potential risks, and expected outcomes. Your informed consent will be obtained before proceeding with any treatment.

5. Payment Policies:

Payment Methods:

  • We accept Visa, Mastercard, Discover, Amercan Express, AfterPay, Cherry Finincing. Payment is due at the time of service.

Insurance:

  • Fit Bella does not accept insurance. Payment is the responsibility of the patient.

6. Minors Policy:

  • Clients under the age of 18 must be accompanied by a parent or legal guardian for consultation and treatment. Parental consent is required for certain procedures.

7. Refund Policy:

  • Refunds are issued at the discretion of Fit Bella and are subject to the terms outlined in our Refund Policy, available upon request.
